Position Title: Case Manager

Job Description

Case managers in the Senior and Adult Services Division provide case management services to individuals age 60 and older and adults age 18 – 59 with physical disabilities. In this role, you will assess, authorize, and monitor the delivery of services to help clients remain in their home environments, reach their highest level of independence and the best quality of life possible.

Responsibilities:

  • Using a person-centered model of care assesses for, authorizes, and monitors services for individuals who are aging and adults with disabilities
  • Provides empathetic, comprehensive, and prompt care to vulnerable clients
  • Utilizes a holistic approach to ensure assessments cover the full range of needs presented by a client, including, but not limited to; medical, behavioral health, and socioeconomic needs
  • Completes required case management documentation within strict timelines based on contractual agreements
  • Utilizes community resources/funding in the most effective, efficient, and responsive manner to benefit the well-being of individuals served
  • Coordinates client care with community partners such as home-care providers, Adult Protective Services, the Public Fiduciary’s Office, Community Paramedicine Programs, hospice providers, and others
  • Performs other duties as assigned, which are consistent with the position and in compliance with agency policies and procedures

Requirements:

  • Bachelor’s degree in Social Work or a related field
  • Two years of professional case management experience with individuals who are aging or adults with disabilities, or other vulnerable populations
  • Must possess or have the ability to obtain a valid Arizona driver’s license at the time of hire
  • Must secure and maintain a DPS Class One Fingerprint Clearance Card
  • Must pass a Central Registry background check
  • Master’s degree in Social Work or related field
  • Case management experience assessing for, monitoring, and coordinating home and community-based services for individuals who are aging, adults with disabilities, or other vulnerable populations
  • Moderate written and verbal bilingual skills in Spanish and English
